Service Description

Attention Patients: This office does not longer take, accepts or participates with any health insurance. This office expects payment at the time of services. This office doesn't provide super bills or insurance billing codes. This office will not process or bill your insurance. This office will not be able to provide you with a superbill including CPT codes. This office will be able to provide you with a copy of a receipt stating that you had “chiropractic services, massage or skin care services”. This office accepts as a form of payment credit cards, debit cards & HSA cards (Visa & Mastercard), Venmo, PayPal, Apple pay, cash and checks . Initial visit and Re-exams one region $139 any other region extra $25 Adjustment only - one region $59 per visit Adjustments and therapies one region per visit $72 any other region an extra $25 per visit
